REPORTS TO: Home Care & Hospice Director

JOB SUMMARY
Provide skilled nursing assessment, observation, teaching and care to patients in their residence. Provide nursing care and case management to home care, hospice, and palliative home care patients. Provide nursing tasks for non-skilled patients as needed. Contribute to patient continuity of care and effective communication among disciplines. Participate on the hospice Interdisciplinary Team (IDT). Supervise other members of the nursing team as requested.

7.Upon orientation complete RN skills checklist and Hospice orientation checklist.

PERFORMANCE STANDARDS
1.Exemplifies good customer service.
2.Makes skilled nursing evaluation visits and skilled nursing visits to home care, hospice, and palliative home care patients. Makes nursing visits to non-skilled patients as scheduled by the Nursing Team Leader, or as required under case management needs. Utilizes the OASIS assessment format as required.
3.Makes initial evaluation visit and skilled nursing visits and initiates individually appropriate preventative, rehabilitative, palliative nursing interventions and coordinates care within physician’s orders per the Plan of Care. Capably performs procedures at RN level of practice. Competently and completely able to assess the physical and psychosocial status of patient and family. Communicates with the physician and other disciplines about changes in the patient’s condition.
4.Provides case management for assigned patients under the supervision of the Nursing Team Leader. Evaluates patient care goals and makes revisions to the Plan of Care as needed, to achieve acceptable outcomes.
5.Identifies patient and family needs; provides and evaluates instruction appropriate for care needs. Includes patient and family in care planning.
6.Utilizes a multidisciplinary approach to patient care and communicates effectively with staff members of other disciplines. Attends and participates on the Interdisciplinary Team (IDT) for patients he/she case manages.
7.Provides accurate and complete point of case documentation of patient visits made and of nursing skill provided. Maintains accurate and up-to-date electronic patient records. Completes daily time logs with accurate visit times, travel time, mileage, and supply charges noted and turns these in per department guidelines.
8.Maintains and improves knowledge of current nursing standards of practice, pursuing competence in new techniques and new regulatory agency requirements. Completes nursing visits supervised by peer RN or supervisor per department guidelines. Completes annual hospital check day, CPR recertification, and education offerings as assigned by Nursing Team Leader. Attends at least 50% of staff meetings, and all mandatory meetings. Obtains education, as needed, for specialized nursing skills. Obtains necessary CE credits as required to maintain RN licensure. Maintains RN licensure for Indiana and Michigan and/or Ohio.
9.Is flexible with day-to-day patient assignments as needed to provide for patient care needs and smooth operation of the Nursing Team.
10.Will rotate on-call responsibility and flex hours worked as needed for vacations and illness, or as patient census mandates.
11.Is knowledgeable of and follows Standard Precautions at all times, other precautions as needed.
12.Participates in department system of Outcome Based Quality Improvement (OBQI) as assigned by supervisor.
13.Abides by department standards for productivity.
14.Accomplishes goals set for self and by supervisor.
15.Maintains confidentiality in all aspects of care pertaining to patients and fellow employees per department standards and HIPAA guidelines.
16.Teaches new staff members as assigned. Supervises other staff members as assigned, including peer visits, supervision of nursing assistants, LPNs, and volunteers.
17.Demonstrates computer skills adequate to complete electronic records as required by the department.
18.Participates effectively in Telehealth.
19.Participates in marketing as requested by supervisor.
20.Demonstrates good customer relations.

QUALIFICATIONS
1.Graduate of an accredited nursing school.
2.Current Indiana RN licensure. Other state licensure as assigned.
3.Current CPR certification.
4.Valid driver’s license and dependable transportation.
5.At least 1-2 years of varied work experience in nursing and be familiar with hospice concept of care.
