STAFF PHARMACIST
REPORTS TO: Director of Pharmacy
JOB SUMMARY
The staff pharmacist, while on duty, will be responsible for the overall operation of the hospital pharmacy when the director of pharmacy is not on duty. Pharmacy responsibilities fall under the Indiana Board of Pharmacy laws and regulations for pharmacy practice. The staff pharmacist must utilize sound medical practice judgments and principles for the safeguard of the patient.
ESSENTIAL JOB FUNCTIONS AND PERFORMANCE STANDARDS
1. Checks;
a. Accuracy of all doses prepared (unit-dose, non unit-dose, IV, topical, I.M., etc.).
b. Patient drug therapy for effectiveness, side effects, allergic reactions, toxicities, and drug interactions.
c. Medication areas on nursing units routinely to insure adequate levels of floor stock drugs and supplies are maintained.
2. Provides;
a. Proper computerized patient medication profiles and billing information.
b. Drug information as needed. Offers to counsel or delegates OFFER to counsel patients with each outpatient prescription that is dispensed.
c. Drug identification services as needed.
d. Inservice education to hospital personnel, physicians, and the public as needed and as staffing levels allow.
e. Assistance in code situations as needed and as staffing allows.
3. Insures;
a. Established policies and procedures are followed.
b. Good techniques are used in compounding extemporaneous preparations and I.V. admixtures.
c. Proper storage and dispensing of medications.
d. All State and Federal drug laws are followed.
e. Review and interpretation of each unit dose and I.V. admixture medication ordered.
f. Identification and follow up as necessary of all relevant and necessary professional interventions.
4. Maintains;
a. Formulary. Assists in drug product and entity selection.
b. Professional competence, particularly in knowledge of new drugs, drug stability, and incompatibilities.
c. Neat and clean central dispensing area.
d. Confidentiality regarding the patient, hospital personnel, and the medical staff.
e. Proper records for controlled substances, professional interventions, and all necessary documentation.
f. Adequate medication stock that is currently dated.
g. Calm and unhurried manner, even under stress.
5. Communicates;
a. Pleasantly and effectively with hospital personnel, physicians, and visitors in writing, verbally and non-verbally.
b. Efficiently and effectively on the phone.
c. With appropriate person when problems are recognized.
d. Observations and information to appropriate members of the health team.
6. Demonstrates;
a. Flexibility in response to unexpected changes in workload, staffing availabilities, or the occurrence of emergencies.
b. Creativity by suggesting new ideas and improvement of existing procedures.
c. Self-improvement by regularly attending inservice programs and outside continuing education programs and by utilizing current literature and reference material.
7. Assumes responsibility for own actions and those of assigned staff.
QUALIFICATIONS
Requirements are representative of advanced levels of knowledge, skills, and/or abilities. To perform in this position satisfactorily requires a minimum of a B.S. degree in Pharmacy. Must be a graduate of an accredited college of Pharmacy. Must have a valid Indiana pharmacy license and maintain 30 hours of continuing education every two years. This position involves light type of work. Ability to stand for long periods of time with some sitting, walking, lifting, grasping, stooping, reaching, pushing, pulling, fingering, talking, and hearing. Visual Activity: A. Worker is subject to inside environmental conditions. Must be able to operate cash register, computer, printer, torsion balance, laminar flow hood, calculator, and a telephone.
