048-15 HHC Medical Social Worker - Home Care/Hospice
HOME HEALTH CARE/HOSPICE MEDICAL SOCIAL WORKER
REPORTS TO: Home Health Care and Hospice Medical Social Worker Team Leader

FLSA Status: Non-Exempt
40 hours bi-weekly, 1st shift

JOB SUMMARY
Responsible for the psychosocial evaluation of social work intervention for Home Health and Hospice patients and families enabling them to deal with the impact of illness. Help assure continuity of care through liaison with hospital discharge planner and other community services agencies.

ESSENTIAL JOB FUNCTIONS
Ability to:
1.Read for comprehension.

3.Assess patients' social and emotional factors and needs and plan and provide care and teaching. Counsel for long range planning and decision-making and for community resource planning. Provide short-term therapy.
4.Process information appropriately (critical thinking ability).
5.Communicate effectively both orally and in writing with patient, family, other staff members, doctors, volunteers, and others as needed.
6.Function independently, work constructively within a complex organizational system, build rapport and work with medical professionals, other social service agencies, patient/family and volunteers.
7.Act as a professional role model, exhibiting behavior, which is indicative of an understanding of social work Home Health and Hospice standards.

PERFORMANCE STANDARDS
1.Exemplifies good customer service.
2.Regular reliable attendance.
3.Ability to get along with others.
4.Make home visit promptly upon referral to complete a psychosocial evaluation on patients and determine appropriate social work interventions needed to assist patient/family in dealing with illness and maximizing their independent functioning. Provide long range planning and referral to community resources as needed.
5.Complete appropriate assessment forms on admission, and follow up progress documentation.
6.Serve as member of Interdisciplinary team. Participates in development of the interdisciplinary plan of care.
7.Keep complete and accurate clinical notes documenting social work interventions identified in the P.O.C. and the progress of the patient/family toward stated goals. Documentation will be done on day of the visit for timely documentation and record keeping policies and procedures.
8.Complete daily time log with accurate visit time, travel time, and mileage and turn into home care billing department as directed.
9.Is knowledgeable of and follows Standard Precautions guidelines, including proper handwashing, proper glove technique and disposal of contaminated waste according to agency policy.
10.Abides by productivity standard of Department (2.75-3 social work visits per an 8 hour shift).
11.Maintains confidentiality in all aspects of care, hold confidential all patient/family info, respect the privacy and dignity of patients and fellow employees.
12.Demonstrates professional ethics and practices the profession of social work as designated in the National Association of Social Workers Code of Ethics.

QUALIFICATIONS
1.Prefer Master's Degree in Social Work from a school of social work that is accredited by the Council of Social Work Education.
2.Licensed as a Social Worker in the State of Indiana.
3.Valid driver's license and dependable transportation.
4.Experience with grief counseling and grief support.

PHYSICAL DEMANDS AND WORKING CONDITIONS
This position involves sedentary type work as well as standing and walking. Direct patient contact subjecting employee to indoor and outdoor conditions, which may involve driving in inclement weather.
