048-29 PHYSICAL THERAPIST'S ASSISTANT - Home Care/Hospice
PRN and On call shifts
Conducts prescribed physical therapy program and provides treatment for medically referred patients. Accepts responsibility for safety and well being of the patient during treatment. Accurately completes all required documentation. The Physical Therapist�s Assistant works under the direct supervision of a licensed Physical Therapist.
ESSENTIAL JOB FUNCTIONS & PERFORMANCE STANDARDS
- Treatment of all patients referred for physical therapy.
a. Makes independent decisions during treatment based on his/her knowledge and assessment of patient's needs.
b. Identifies changes in patient's condition and needs and communicates those changes with the physical therapist.
c. Assesses the response of patients to teaching, and documents on the patient's medical records that the patient and /or family understood instruction.
d. Recognizes the response of patients to treatment, possible contraindications to prescribed treatment, and documents on the patient's medical record.
e. Administers treatments to patients utilizing neurodevelopmental techniques, muscle reeducation, gait training, massage, soft tissue mobilization, ultrasound and electrical stimulation to decrease pain and increase function.
f. Instructs patients in use of assistive devices/prostheses on level and environmental surfaces, to increase independence in activities of daily living.
g. Instructs patients/caregivers in exercise programs for restoration of joint and muscle function, transfer techniques and activities of daily living.
h. Discusses patient treatment, care, and progress with physician and supervising physical therapist.
i. Documents treatment plans and goals, and maintains daily progress notes, and upon completion of treatment, summarizes progress, treatment and expectations in the discharge summary.
j. Communicates effectively with inter-department personnel for continuity of care.
- Actively participates in management of the department.
a. Is responsible for filling out incident reports.
b. Attends monthly department meetings.
c. Takes part in inservices.
d. Keeps up to date on physical therapy procedures, attends continuing education courses, makes recommendations for improvements in education courses, makes recommendations for improvements in present department clinical techniques, procedures and programs, and keeps professional staff informed about these new techniques.
e. Assists and directs others assigned to these activities to maintain a neat, orderly and safe work area.
f. Assists director in updating department procedure manual.
g. Assists director in department quality review program.
1. Must be a graduate of an accredited physical therapist�s assistant program.
2. Must be currently licensed as a physical therapist�s assistant in the State of Indiana or must be eligible for national examination given by the A.P.T.A.
3. Must be CPR certified.
