It’s easy to sign up.

To get started, simply call your care team’s office for an activation code. You can also contact the Cameron MyChart Registration line at 260-667-5655. The line is open Monday through Friday from 8 am to 4 pm.

It’s simple to use.

Once you sign up for Cameron MyChart, it is easy to use. You can connect on your mobile device, tablet, laptop or desktop computer any time of day or night. You also have access to help through Cameron MyChart information line at 855-853-0001.

It’s safe and secure.

We take multiple steps to ensure that your health information is kept private and secure using Cameron MyChart. Access is controlled through a series of activation codes, personal usernames and passwords. All of your information, including your name and email, is given the same level of privacy and security as all your health records, so it will never be shared.

When you arrive, check-in through Cameron MyChart to minimize wait times. In addition, if you enable alerts on your phone, the MyChart app will send you appointment reminders.
